Here are the steps to be seen via telehealth:
- Make sure you have a smartphone, tablet, or computer with video capability.
- Make any required payment of copays, coinsurance, or deductibles prior to your scheduled appointment; otherwise, your appointment may need to be rescheduled.
- You will receive a link for your visit. If you have not received a link, please contact us at (480) 716-8114.
- You will be sent to the virtual waiting room until the start of your session.
- Your provider will open the telehealth session once he/she is ready to start.
- If you are experiencing technical difficulties, immediately contact our office by calling us at (480) 716-8114.
To receive telehealth services, you understand, acknowledge, and agree to the following limitations, risks, expectations, and responsibilities:
- Establish and maintain confidentiality during my electronic communication during the treatment session.
- Confirm my identity at the start of each telehealth treatment session.
- Fulfill any financial responsibilities (current or delinquent) prior to scheduled telehealth treatment sessions.
