  • Currently, we DO NOT have a contract with any MEDICAID Plans.
  • Some services may require insurance pre-authorization to be covered.
  • Accepted insurances listed above are subject to change without notice.
  • NOT ALL services or treatments are covered by insurance.

PAYMENT/NO-SHOW/SAME DAY CANCELLATION POLICY

  • NON-PAYMENT: If your account is over 30 days past due, you will receive a letter electronically or by mail stating that full payment is due upon receipt. Partial payments will not be accepted. Please be aware that if a balance remains unpaid, your account may be referred to collections and you and your immediate family may be discharged from this practice. If this is to occur, you will be notified by mail or electronic letter that you have 30 (thirty) days to find alternative medical care.
  • MISSED APPOINTMENTS: Our policy is to charge for appointments that are not cancelled at least 24 hours ahead of the said appointment, otherwise the following fees will apply:
    1. NO SHOW / CANCELLATION Within 24 hours – $100 Intake, $50 Follow-Up. (This charge is not for a service but rather for a missed business opportunity).

FEES

  • Prior to completing any LOA / Disability / Accommodation paperwork, patient must understand the following:
    1. Must be an established patient (greater than 6 months), we do not complete the above mentioned paperwork for New Patients.
    2. Our providers DO NOT complete long-term disability paperwork.
    3. Our providers DO NOT complete SMI paperwork.
  • LOA / Disability / Accommodation documents and/or letters – $35 (per page)
  • Medical Records Request (Coordination of Care with Provider/Facility/Hospital/Insurance) – NO CHARGE
  • Medical Records Request (Self/Guardian, Law Firm, Disability Agency, Other) – $40 for the initial 100 pages, then $0.10 per page thereafter.

NOTE:

  • We DO NOT offer sliding scale payments.
  • Fees are subject to change without notice.
  • Telehealth visits not covered by insurance will be subject to the cash pay rate.
  • Your Credit Card information will be required prior to receiving any services.
  • In accordance with the No Surprises Act, we will make a good-faith effort to notify patients when their services exceed $400.
